I authorize treatment for my child, under the direction of medical or emergency personnel, in the event of a medical emergency. I agree to be held financially responsible for any such treatment. *

I agree to hold Iron Mill Church, along with it’s employees and volunteers, free from any claims that may result from any injury or illness caused by my child's willful negligence. *

I understand the risk of my child contracting COVID-19 while participating in any Iron Mill Kids activities. *
